Chamberlain NR-511: Differential Diagnosis & FNP SOAP Note Blueprint
Avoid faculty point deductions on your FNP clinical SOAP notes. Step-by-step blueprint covering OLDCARTS subjective data, pertinent negatives, ICD-10 differential ranking, and evidence-based pharmacotherapy.
The 3 Critical Flaws That Cause SOAP Note Deductions
In Chamberlain NR-511 and NR-601, clinical faculty expect graduate-level diagnostic reasoning. The three most common deductions are: (1) differential diagnoses lacking official ICD-10-CM codes, (2) missing pertinent negatives in the Review of Systems (ROS) and Physical Exam, and (3) non-guideline concordant pharmacotherapy.
When documenting patient encounters, your Assessment section must present at least three prioritized differential diagnoses with current ICD-10 codes and an evidence-backed rationale justifying why each was considered and ranked.
- Complete OLDCARTS analysis for Chief Complaint (Onset, Location, Duration, Characteristics, Aggravating/Relieving, Radiation, Timing, Severity)
- Explicit documentation of pertinent positives and pertinent negatives
- At least 3 differential diagnoses with valid ICD-10-CM codes (Primary, Secondary, Tertiary)
- All 6 legal prescription components for every medication (Drug, Dose, Route, Frequency, Duration, Dispense/Refill)
- Direct citation of current Clinical Practice Guidelines (CPGs) within the management plan
